FAITH FORMATION PROGRAM GUIDELINES
Students are to
attend all classes and are to report to their classroom on time. If
an absence is unavoidable, please call or email the Faith Formation
Office to report the absence.
● If your child is absent, parents are to consult the

● There is a $20.00 charge to replace lost textbooks.
Parents are asked not to take students from class before dismissal
time. Requests for early dismissal must be made through the
Faith Formation director. Early dismissals will not be permitted
unless an “Early Dismissal” slip is obtained from the Faith
Formation Office and presented to the catechist at the beginning of

Disruptive classroom behavior will not be tolerated. Parents will be
contacted by email or telephone if a student is sent to the office
for disruptive behavior. Repeated behavioral issues will result in a
meeting with a member of the clergy and the Director of Faith
Formation to determine the best course of action in a given
